This case report describes a live birth resulting from intracytoplasmic sperm injection (ICSI) of spermatozoa retrieved by microdissection testicular sperm extraction (micro-TESE) into oocytes produced from hCG-primed in vitro maturation (IVM) cycles. In the IVM treatment, a total of 30 oocytes (one mature and 29 immature oocytes) were retrieved. Following IVM, 9 oocytes had matured. A total of four oocytes were fertilized after ICSI with the husband's micro-TESE spermatozoa and four embryos were transferred into the uterus on day 3. A healthy boy weighing 2500 g was born at 35.5 weeks of gestation.

Key words: ICSI • In vitro oocyte maturation • microdissection testicular sperm extraction • non-obstructive azoospermia • polycystic ovaries
